This questionnaire will give your physical therapy provider information about how your neck condition affects your everyday life. If you are suffering from neck pain and have scheduled or plan on scheduling your first appointment for the PT evaluation, please fill out the All Patient form as well as the Neck Index form.
If you are suffering from back pain, please fill out the Back Index form and the All Patient form. The Back Index will give your physical therapist insight how your back problem is affecting your daily routine, work, and other activities.
If your condition is related to your hand, arm, or shoulder, please fill out the Disabilities of the Arm, Shoulder, and Hand form (along with the All Patient form). Here we will be measuring the difficulty of a list of common activities as a result of your condition. We are interested in knowing more about the type of pain or discomfort you are experiencing and the severity of your symptoms.

General Orthopedic Conditions
Orthopedic physical therapy treats injuries and conditions of the musculoskeletal system. These conditions can occur at any age and can cause pain or discomfort anywhere on the body.
Body Imbalance
Physical therapy for body imbalance treats a wide variety of balance and vestibular disorders. The goal of this type of treatment is to improve balance, decrease dizziness, and minimize falls by restoring normal vestibular function and posture control.
Rotator Cuff Syndrome
Rotator Cuff Syndrome is a general condition of the shoulder usually secondary to deficits of glenohumeral and scapular humeral biomechanics. We identify and address the specific dysfunction in order to return the patient to full function.
Carpal Tunnel Syndrome
Carpal Tunnel Syndrome is often cause by overuse and repetitive motions. It is characterized by stiff wrists, impaired mobility of the wrists and fingers, and severe prolonged pain. Treatment for Carpal Tunnel Syndrome typically begins with anti-inflammatory medications. If the swelling or discomfort does not improve, physical therapy may be suggested. The goal of physical therapy is to strengthen the injured muscles to prevent pain and fatigue. Active Release Therapy (ART) is also extremely effective in treating Carpal Tunnel Syndrome.
Sciatica
Sciatica is a term used to describe pain that is often felt in the buttock that may travel into the back of the leg. This pain may arise from pressure on the sciatic nerve roots in the spine, tightness in the buttock muscles, or a combination of both. Joint dysfunction in the lower lumbar and sacral spine can cause repetitive irritation to nerve roots through normal movement such as rising from a seated position. Piriformis Syndrome can also cause sciatica. The piriformis is a deep hip muscle which when shortened or overused can pinch the sciatic nerve. Severe, constant symptoms below the knee such as complete numbness or weakness, require immediate medical attention and may need more aggressive intervention.
Lower Back Pain
Physical therapy treatment for lower back pain often involves a wide range of techniques including heat therapy, ultrasound, massage, mobilization, exercise, and education about posture and body mechanics. Stabilizing and strengthening the lumbar spine (specifically lumbar multifidus and transversus abdominis) through a lumbar stabilization program is also an important part of a rehabilitation program for a patient suffering from lower back pain.
